Adult Acne Is Different….And It Deserves a Smarter Approach

And it’s different.

Different triggers. Different patterns. Different treatment strategies.

The good news: we have more effective tools than ever, especially when we combine medical therapy with in-office treatments like lasers and chemical peels.


Why Adult Acne Happens

Adult acne is rarely just about “clogged pores.” It’s usually a combination of:

1. Hormonal Fluctuations

Even in your 20s, 30s, and 40s:

  • Androgens stimulate oil production

  • Breakouts often appear along the chin, jawline, and lower face 

This is why many women notice flares:

  • Around their menstrual cycle

  • After stopping or changing birth control

  • During periods of stress


2. Stress and Inflammation

Stress increases cortisol, which:

  • Triggers oil production

  • Increases inflammation

This leads to deeper, more cystic, tender breakouts—very different from teenage acne.


3. Skin Barrier Disruption

Overuse of:

  • Harsh cleansers

  • Exfoliating acids

  • “Trending” skincare

…can actually worsen acne by irritating the skin and increasing inflammation.


What Makes Adult Acne So Frustrating

  • Breakouts are often deeper and slower to resolve 

  • Higher risk of post-inflammatory hyperpigmentation (dark marks) 

  • Greater chance of scarring 

  • Skin may be simultaneously acne-prone and sensitive 

This is why a one-size-fits-all approach doesn’t work.


The Foundation: Medical Treatment

Before we even talk about lasers or peels, we build a strong foundation:

  • An effective wash or topical Benzoyl peroxide or Sulfacetamide

  • Hormonal therapies when appropriate

  • Oral medications for more severe cases

But often many patients plateau.


Where Lasers and Peels Change the Game

Think of these as accelerators and refiners, not necessarily replacements for medical therapy, but powerful complements.

1. Chemical Peels: Resetting the Skin (we have over a dozen types in our office)

Superficial to medium-depth peels can:

  • Unclog pores

  • Reduce oiliness

  • Fade dark marks faster

  • Improve overall skin texture

Common types we use:

  • TCA, Salicylic acid peels (excellent for acne-prone skin)

  • Glycolic acid peels (for texture and tone)

  • Mandelic acid (for pigmentation)

Why they matter:They help break the cycle of congestion and pigmentation, especially when topical treatments alone aren’t enough.


2. Laser and Light-Based Treatments: Targeting Inflammation and Bacteria

Modern devices can:

  • Reduce acne-causing bacteria

  • Calm inflammation

  • Decrease oil gland activity

  • Improve redness and early scarring

Depending on the device, we can target:

  • Active acne

  • Residual redness

  • Texture changes from prior breakouts

Why they matter:They address the biology of acne at a deeper level than topical products alone.


3. The Real Advantage: Combination Therapy

This highly effective when guided by a skin expert.

When we combine:

  • Medical-grade skincare

  • Prescription treatments

  • Peels

  • Laser/light therapies

Purpose:

  • Clear acne faster

  • Reduce reliance on long-term antibiotics

  • Prevent and treat pigmentation

  • Minimize scarring

This is not about intensity but rather about using our expertise to formulate the right combination.


Why Guidance Matters

Lasers and peels are powerful, but not interchangeable.

The wrong treatment can risk Irritating acne, worsening pigmentation, delay healing

When guided by a dermatologist, we tailor:

  • Type of peel or laser

  • Timing relative to your medications

  • Frequency and intensity

This is especially critical for:

  • Skin of color

  • Sensitive skin

  • Hormonal acne patterns


Our Typical “Modern” Acne Plan

For many adult patients, a plan may look like:

  • Daily topical regimen (retinoid + targeted treatments)

  • Periodic chemical peels (every 4–6 weeks)

  • Laser/light sessions for inflammation or redness

  • Hormonal or systemic therapy if needed

Each piece supports the others.

Adult acne is no longer something you just have to “live with.”

The most effective results come from:

  • Understanding your specific acne pattern 

  • Using evidence-based treatments 

  • Strategically incorporating our in-office procedures
